Nourish Lactation Consulting works with Aetna, Anthem, Blue Cross/Blue Shield, Cigna, UnitedHealthcare and Multiplan.

If you *or your baby* has Anthem, Blue Cross/Blue Shield, or United Healthcare health insurance: please fill out two things:  
If you have coverage, you will pay nothing out of pocket (other than a travel fee for a home visit) and the Lactation Network will handle the paperwork and bill your insurance plan directly. 

If your insurance plan works with the Lactation Network:
  • Office and telehealth consults are free
  • Home visits will incur a travel fee dependent on your location.

Nourish Lactation is partnered with the Lactation Network, which can bill most insurance plans directly so that you get six lactation consults at no cost to you.

Aetna

I am in-network with Aetna. Aetna usually covers up to six lactation consults, though depending on your plan you may have a copay for you and/or for your baby. There is an additional travel fee for a home visit that is not included with insurance.

If you have Aetna as your primary insurance, please fill out this form when requesting a consult. 

Please note: if you choose to work with a lactation consultant who is not in-network with Aetna, you will pay out of pocket and it is unlikely that Aetna will reimburse you for the full cost.


Cigna
Other or no insurance

If you do not see your insurance listed, please email me a picture of the front and back of your insurance card as there is a chance that your insurance will still cover your visit. If I can't take your insurance, I can provide you with a "superbill" receipt you could submit to your insurance to try to get reimbursed. Before the consult, I can provide you information that you can discuss with your insurance company to see if you will be reimbursed. I can also provide you with a receipt for HSA reimbursement.

I offer a discount for Medicaid recipients.

Initial consult:
  • Home visit: $225 for approximately 1 hour and 45 minutes. If further than 15 non-mountain road miles away from my Boulder office there is an additional cost, dependent on distance.
  • Office visit: $180 for an hour and a half.
  • Virtual visit: $150 for an hour and a half, reduced for shorter consults.
  • Prenatal virtual visit: $90 for an hour.
Follow up consults:
  • Home visit: $150 for an hour and 15 minutes. If further than 15 non-mountain road miles from my Boulder office there is an additional cost.
  • Office visit: $100 for 55 minutes. 
  • Additional home or office visit time: $25 per 15 minutes.
  • Virtual visit is $50 for 30 minutes.
  • Additional virtual visit time: $20 per 15 minutes.
